Job Description

COMMUNITY HEALTH NURSE

Reference No. TWT-HEAFAM-CHN-0219

 

Purpose: Reporting to the Senior Nurse, the Community Health Nurse is to provide communicable disease control, provide family wellness services, and educate members through community outreach. This is a full time permanent position that works primarily in the office with moderate local travel required.

 

Responsibilities:

·         Administer recommended immunizations to community members

·         Work collaboratively with Local and Provincial Health Authority to respond to communicable disease outbreaks such as tuberculosis

·         Assess, diagnose and treat sexually transmitted infections

·         Provide basic counselling,  advocacy, and education to clients and community

·         Provide perinatal education and support to families

·         Provide reproductive care including contraceptive management

·         Provide health promotion education regarding a wide variety of health concerns

·         Ensure accurate timely documentation, which includes organizing assessments and client care plans in electronic medical record

·         Ensure culture safety of all clients when receiving services

·         Conduct neonatal and child development assessments

·         Conduct home visits for clients  

·         Collaborate with community partners

·         Refer to internal and external resources when appropriate

·         Coordinate and assist community events to promote health and wellbeing

·         Liaise with children and youth outside the health center

·         Participate in outreach programs to promote harm reduction and respond to wellness needs

·         Other related duties as required

 

Required Education and Experience: 

·         Bachelor of Science in Nursing required

·         Member of the British Columbia College of Nursing Professionals

·         2 years community nursing experience required

·         Certification in First Aid / CPR Level 1

·         Certification in BCCDC Immunization Competency

·         Certification in Sexually Transmitted Infection and Contraceptive Management preferred

·         BC Drivers’ License and reliable vehicle

·         Criminal Record Check

Required Skills, Knowledge, and Abilities:

·         Understanding of the impact of intergenerational trauma, its complexity; and willingness to learn about Cowichan Tribes history, culture, traditions and values

·         Knowledge of internal and external community resources

·         Community-centered, team-orientated, and ability provide excellent service to the community

·         Well-developed organizational, written, oral, and interpersonal skills with the ability to multitask
