Job Description

CLS Adults is a proud team of long-term staff working with clients who live with various diversabilities. Program seeks a reliable casual to cover vacations, sick days, and extra support needs for our outreach team. Our clients receive services in the community from 9:00 a.m. to 6:00 p.m., Monday to Friday. Tasks may include grocery shopping, fitness activities, social events, emotional and behavioural support and life skills coaching, on a one-to-one basis with individual clients. We do not administer medication or provide personal care for clients. Casual staff can expect shifts between 3 and 7 hours and can have between 6 and 24 hours offered each week. Unfortunately, no hours are guaranteed, but we try hard to offer as much work as we can to casual staff, as they are an important part of our team.

Job Summary

Provides outreach support to individuals with developmental disabilities to enhance their success in day-to-day community living. Worker will offer life skills training, employment skill development, emotional support, social opportunities, crisis intervention, education, and advocacy on a one-to-one basis or in a group environment.

Wage/Salary plus 10.2% in lieu of statutory holidays and vacation, per Collective Agreement

Work Hours  Position may require occasional flexibility evenings and/or weekends

 Key Duties, Responsibilities

Plans, organizes and implements educational, recreational, social, employment and life skills-based programming based on individual goals

Monitors, reviews, and evaluates progress made by client; modifies as needed

Acts as an advocate for client

Completes client intake assessments

Creates Individual Service Plan (ISP) from evaluation of client

Ensures client and their parent(s)/caregiver(s) participate in developing ISP

Provides clients with information on employment opportunities; provides support and skill development in areas related to obtaining employment

Handles potential/emerging crisis situations

Identifies and/or makes referrals to various services in community to further support identified client needs

Liaises with other agencies, professionals, government officials and community as necessary; contacts potential employers to develop relationships

Works with team and Program Manager to address needs of program and clients

Participates in agency activities as required

Maintains complete, legible and organized documentation on client activities and all other program-required documentation

Meets documentation timelines established by program and CARF Accreditation Standards

Maintains best practice standards as identified by Phoenix Policy & Procedure Manual and Health & Safety Handbook

Maintains cleanliness of worksite, including agency vehicle(s) used by program

Other related duties as assigned by supervisor

Education, Training, Experience

Community Support Worker diploma or equivalent combination of education and training

1 year previous relevant work experience required; direct program delivery experience in Community Living sector, demonstrated working knowledge of appropriate community supports

Good written and verbal reporting skills

Understanding concepts and values of community living and mandate of CLBC

First Aid with CPR-C required

Job has physical demands such as walking, lifting, etc.

Valid driver’s license & use of vehicle with business insurance required
